Allow Multiple Entries
I know I must be missing something simple. I'm working on creating a Lookup field - so I have created a form for users to add in Locations that we will be looking up. - Simple form with just one field - but when I create a page for that form it only allows me to do one entry and then gives me

Now I need to redo the form to add the second location. I know I can add this as a CSV but I need my user to be able to add locations as we move forward.
What am I missing? I have not selected anything that would limit entries to one form per user - double checked that.

If you reload the page, you should be able to add more, but I think you are wanting to be able to do multiple submissions in a row without a page refresh.
