Can I do this with Formidable Forms?

I am looking for a wordpress plugin where I can have my clients fill out a contract online and submit it to me.  When I receive the contract, I don't just want the fields, I need to see the whole contract as a document.  I am attaching a copy of my contract to make it more clear what I am trying to accomplish.  Is this possible with Formidable?

You can accomplish this by using a third-party plugin (https://www.formidablepro2pdf.com/). I've used it for a few projects and it works great. You have to create a pdf template and map the fields in your form to the fields in the template, but once that is finished you will get dynamically generated pdf forms sent to you whenever someone fills out your form online. You will need Formidable Pro as well as pdf editing software to create the pdf template.

The team at PRO2PDF can also create a Contract for Puppy Sale template for you at a cost of $10-$15 if you don't have time to learn how to use the plugin. Also, it looks like the free version of Formidable Forms may be used for your form. You can submit a request here.

Speaking from experience, $10-$15 would be money well spent to have them just create the template for you.
