Is there a way to create nice selection boxes like this:

https://app.brandquiz.io/template/customer-satisfaction-survey

(See question 2)

With a combination of CSS/JS/Jquery, yes. Time to lay it all out here for free? Probably not.

Here is a start point maybe... https://www.techmavenconsulting.com/2015/05/26/formidablepro-fancy-radiocheckbox-buttons/

@Bobby - thanks. That's one complicated solution for something that should be built in IMHO. I assumed they'd have an easier option.

I'll probably rather spend a $100 bucks and get something that looks 10x better and has all the templates.

When I have some time I'll delve into that link if the other solution runs into issues.

Thanks for the link though.

I agree. Alternative modern field type styling is a valuable missing feature. I hope they get to it one day soon.

They've nailed just about everything else with form data capture and display, but there are some styling and typical features you would think exist, but do not. Formidable stands on both sides of the fence, giving you the power to do what you want in almost any way, but misses the mark on a few minor things.

I realize this may not be minor to you, but if you know the entire plugin, you would see this as a minor issue.

I agree and use Formidable extensively with many of the features.

For me it's not minor as it impacts conversions. I guess it's the same for all users. Buttons convert better than typing stuff out on mobile. It's a pity they view it as minor.

I've had to get around it using custom buttons.

But it really should be easier and more robust. They also don't let you auto save an entry after someone has put in their email! Another big weakness when it comes to conversions.

Anyways, was hoping for a quick solution. I know Brandquiz has a 24 hour LTD deal coming out which I'll snag. Looks like a SAAS form based product may be the future. :-)

You are right about that. It is lacking in supporting leads/conversions in the regards you stated. The only way I can envision capturing an email at this point is by linking two forms, one that simply captures the email address and another with form data. Then when a user enters their email in form 1 and clicks "next", the emails is pushed through the system and passed as a parameter which then redirects to a page with the actual form data to be collected.

A lot of unnecessary work if it could be done a different way.

@Bobby - that's exactly what I have for longer forms. It's a nightmare but it's the only workaround.
