Creating Auto Response

I have Formidable (Lite) and I am trying to create an Auto Response that is sent to the individual that filled out the form. My initial form is working but I can get the Auto Response form to work. Attached is a screen shot of the response email I have created. Does anyone have an idea of why this is not working?

Hi David,

In the 'To' field you need the shortcode for the recipients email address.

At the moment it looks as though you have [19] (which is their first name) and then Name<name@email.com>

You'll need to remove all of that and just have the email address field shortcode (assuming you are capturing their email address int he form?).
