Dynamic targeting of email

I was able to accomplish what I needed -- use the same contact form on multiple pages, but send the resulting email to a different address based on the individual pages.
Just to clarify:
1) Add a custom field to the posts/pages containing the form. In my case, I wanted the form results to send to a specific email address, different on a page-by-page basis, so I added a "target_email" custom field with the value of the actual target email for that page.
I also wanted the company's name to appear in the subject line, so I added a 2nd custom field "distributor_name" with the value of the company in the value field.
2) In the form, I added an invisible field, called it "Target Email" with a default value of [post_meta key=target_email] and an invisible field called "Distributor Name" with a default value of [post_meta key=distributor_name].
