Employment application needed

I'm looking for someone to build an online employment application using Formidable. Currently the application is 4 pages. Would like to convert to an online version. I didn't see a template to get this going so I'm wondering if someone would like to take on this task as a paying job? Thank you!

They decided not to use it because they want to see their applicants face-to-face. You can have the XML and use it as a template if you want.

For some reason, the file didn't get attached to my last response.

I built this one with hidden fields that depend on the responses of questions before them. For example they can list anywhere from 1 - 3 jobs and so on.
