Enter a "Code" and allow people to enter info and then show prior entries

I would like to create something similar to kindnesstracker    dot    org  where a person can enter a code into the first entry,after doing so it would provide the options to make an entry or show the user the prior entries.

Prior entries would include their name, city, state, what kind act they received and then it would show a list of all entries for that specific code.The codes will be the variable data on the RAKM cards that we print - servedaily.com/rakm

Is it possible to do this with Formidable?

So basically you'd create a single line input field and call it "card number". You can then build a view to show entries created from the form that contains this "card number" field and add a filter to show only entries with that card number. You would create a search form that would translate the search variable to this view that will then filter the entries with that unique "card number". You can use the content box for no entries to add a form for entering new RAKM's. Easily said, a bit more complicated to do.
