Form Submissions With Images Creating Multiple Images Sizes For Each Photo

I use a form for our readers to submit their pictures. Just noticed that images submitted are being created in multiple resolutions. In attached screen cap, the original submitted image is 006.jpg and all the other files are being created by Formidable. I've had the wordpress media settings all set to 0 pixels for the last couple years (checked, they still are), where could these settings be coming from and how can I disable the thumbnail generation? I also have NextGen gallery installed but did not see thumbnail settings anywhere that could cause this.
