Hello,

I want a  grid with checkbox and level, I have attached this image,

Here I found some links, but its not understandable.

Checkboxes and Radio Buttons

Could any one help me out.

The labels on the top would be custom HTML.

You can spread the checkboxes apart with CSS. You may have to add custom classes to the Customize HTML page in the form's settings. This would be an ideal situation to use CSS grid.

If you're not familiar with programming at this level, you may want to consider hiring a developer for a couple of hours. Explaining how to do this in code would take longer than just doing it.
