Help with Data Driven Dynamic Values in Select Dropdown

I want to populate a dropdown (select field) with options from a database table located within the same wp database.

When the page is rendered and it has my formidable form on it, I need to populate the dropdown box with values returned from a query that was run at the top of the template.

How do I go about injecting my data into the dropdown as options of a select field?

I know how to do do everything on the php side of things and can get my query results to the template. What I do not know how to do is inject the results as options.
