How can I include the formidable login form in the site header or on a separate bar on top of the page?

I lot of what you are asking depends on your theme and how the theme developer implemented widget areas.

You can us the Formidable Form widget in a widget area on in your theme. If a widget area or hook doesn't doesn't exist in the target area, you should contact the theme developer to ask how to create one.

You can also use the form's shortcode as above.
