We have a site with hundreds of products that all use the same quote request form. Is there a way to display what page the form was submitted from?

I don't understand your question. Display the page where? What exactly are you trying to accomplish? Please explain the workflow.

Sure. You could add a hidden field to the form and then use JavaScript to copy the page ID into it. A shortcode that fetches the page ID on load and used as a default value might also work.
