Need some help with a short form that leads to a longer form
I was hoping someone could help me out with something ... I've had Formidable Forms for awhile but I haven't done anything more than the most basic questionnaire. The thing is, my questionnaire is quite long and I'm afraid it is scaring people away when they see it on my website. It is essentially a contact form for potential clients, but it has a lot of more detailed questions, should they want to give me additional information.
What I would like to do is, have a very basic form, asking for Name, Email address, and maybe one other item. Then when the user clicks Submit, I would like it to open up with additional questions, and ask the user if they would like to provide additional information. If not, then it emails me the basic information they already submitted, so I can contact them and move on from there.
So if they decide to answer any other questions, I would get an email with the longer form filled out. If not, I just get an email with the basic information.
I'm pretty sure Formidable can do this with the conditional logic, but I have no idea how to start out doing it. Can anyone point me in the right direction? Thanks!

Hi Blake,
Here is one way of doing it.
1. After "Name" and "Email" fields, add a radio button with options Yes and No and add a label "Would you like to provide additional information?" or something similar.
2. Next, add a "section" field after the radio buttons and then move all other questions inside the section.
3. In the section's conditional logic, configure to show the section only if the radio button is Yes

Thank you Sujeevan! I actually did something similar, although I didn't use the Section field ... I guess that would have been more efficient, but I added the conditional logic to every question after the radio button.
Thanks for your help!
