I'm using formidablepro-2.02.13

Look at the attached graphic.  the name is clearly Michelle TEST.
and the email is michelle@design4dot.com
but if you look at the FROM: in the email it is Michelle <TESTmichelle@design4dot.com>

The form action was created using your shortcodes.  I'll send it separately since I can only submit one graphic at a time.

Hi Michelle,

Thanks for submitting your issue to the community. I did want to let you know that multiple files can be uploaded in one instance of a file upload, simply select multiple files before clicking on "upload".  Sorry this wasn't clear. We'll update the form to reflect this.
