Terms of Use integration

Hi all,

I have an issue that I hope someone can help with.  I'm trying to use the "terms of use 2" plugin with Formidable forms,  both from the same developer.

When I install the Formidable plugin the area in the "terms of use 2" plugin that I pick the Formidable form to insert the terms into gives the following error.  I'm not sure if the plugin is outdated or has replaced with another function.

Any help would be greatly appreciated.

If you put the terms of use onto a page that doesn't have a form what happens?
