This example will allow you to load form's email settings with AJAX; this can be especially helpful if you have a lot of email settings or notifications.

By default, there is a blank option in a Dynamic field dropdown. You can change the label on that option with the code below.

Javascript will not be processed by default, but you can use a bit of added code to strip the javascript before save. No changes are needed to this example.
